Receive support from a dedicated Care Coordination team for
scheduling and documentation.
Provide high-quality physical therapy services in patients' homes
across a 40-mile radius.
Responsibilities include supervising assistant staff, communicating
with patients and agencies, completing evaluations and plans of
care, and attending meetings.
Requirements: current licensure, Bachelor's degree in Physical
Therapy, ability to travel, strong communication skills, home
healthcare experience preferred.
