Agape Care Group is seeking a compassionate and dedicated Social Worker to join our team in the Southeast region. This vital role involves working in hospice and palliative care, providing personalized support to patients and their families during the most significant and sensitive times of their lives. Our Social Workers are crucial in upholding our mission to enhance dignity and comfort while ensuring the highest quality of life for our patients through tailored care plans. Assess patients' and families' needs and provide psychosocial support. Develop and implement personalized care plans in collaboration with other healthcare professionals. Assist patients and families with navigating healthcare systems and understanding treatment plans. Provide counseling and support to help patients and families cope with emotional, social, and financial challenges. Document and maintain accurate patient records in accordance with healthcare regulations and company policies. Coordinate with community resources to support patients’ and families' needs beyond the clinical setting. Educate patients and families on hospice and palliative care processes. Strong interpersonal and communication skills Ability to handle sensitive information with confidentiality Knowledge of healthcare systems and resources Adept in crisis management and intervention techniques Proficiency in clinical documentation and record-keeping Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) or eligible for licensure CPR certification preferred
